Position Summary
The Physician Case Manager role assists members in navigating their medical journey including, interviewing members for case evaluation, preparing their medical information for assessment by experts, developing questions to ask experts, and regular on-going contact with members to assist through the process. 

The ideal candidate for this role is a physician who thrives in a patient advocate role and who flourishes in an environment of curiosity and discovery to best help our members on their medical journeys.

We are seeking candidates who are available to commit to 15-20 hours per week to this position.  Hours can vary from week to week, dependent on member needs.  Physician Case Managers are 1099 independent contractors and non-benefited.

Responsibilities
1) Obtains a comprehensive health history by leading members through a systematic and dynamic intake assessment to capture all relevant data about their current condition and health history.

2) Directs patients to the most appropriate service line based on their medical condition; sets appropriate member expectations for each process.

3) Works in partnership with the clinical team in support of case development.
Support expediting procurement of patient medical records.

4) Write comprehensive clinical summary and present expert physician findings.

5) Communicate with patients in a caring and compassionate manner; communicate with EMS team in a courteous and professional manner.

6) Track all communication with members in Salesforce (training provided).

7) Adhere to and abide by all regulations, policies, work procedures and instructions.

Minimum Requirements

-Board certified in specialty/subspecialty (ABMS or AOA)

-Active state medical license(s)

-Active Federal DEA certificate/number

-No open malpractice claims

-No open licensing board complaints or a probated or restricted license

Skill Requirements

-Possess excellent written and verbal communication skills

-Demonstrate effective organization and time management skills

-Belief in the role of physician as both clinician and patient advocate

-Able to work independently and be flexible

-Able to maintain confidentiality

-Proficient in technology, including Microsoft Office suite
